Medical declaration

To be covered for the valuable death in service and ill health benefits, all new members must provide information on any pre-existing medical conditions. The process for this is explained below, but please remember that the medical information you provide must be as at the date that you joined the Scheme.

Step 1 - check the list below

  • Multiple Sclerosis
  • Motor Neurone Disease
  • Any disorder of the kidneys
  • Stress, Anxiety or Psychiatric illness (for which you have received treatment)
  • Diabetes
  • Any disorder of the heart, including murmur, stroke, chest pain, palpitations
  • Any blood circulatory problem including haemorrhage or clots
  • Any form of cancer, including leukaemia, tumour or growth, including moles that a healthcare practitioner has advised may be cancerous
  • HIV

Step 2 - contact pensions

If you have been diagnosed with any of these conditions, you need to call the team on 0800 090 2267, they will take your details over the phone and send you confirmation in writing of any restrictions.

If you have never been diagnosed with any of these conditions, you need to send an email to our dedicated mailbox lifecoverconfirmation@abfoods.com, confirming the following information. You will then be issued with a letter confirming that you’re covered:

  • Your name
  • Your member number (or your date of birth if you don’t know your member number)
  • The statement - “I do not have any of the medical conditions listed”

It is important to note that a false declaration could lead to non-payment of benefits at a later date and it is your health condition at the time you joined the Scheme that applies.

All calls to the above phone number are recorded and the information taken will be used solely to assess the provision of benefits for death in service and ill health retirement.

If you joined the DC Section immediately following flexible retirement or following the transfer from CEVA, Holgran, or Fleming Howden, you should already be covered.
